Modern software is becoming increasingly better at arranging packets in the correct order (often through slight time delays which allow them to be rearranged) and covering up 'dropped' packets. VoIP phone conversations are therefore more audible now than they were in the early days of VoIP phone systems.
As I said, you can use your computer microphone and speakers to make and receive conversations, but you can also get a dedicated VoIP phone if you prefer. It's all a matter of choice and does not significantly affect the services you can have.

* Call forwarding
* Voice mail
* Conference calls
* Call transfer
* Group pick-up
Once you have your box, any calls you make to another VoIP are free–anywhere. For calls to non-VoIP phones there is a charge, but this is generally lower than normal land-line charges.

You can also buy a VoIP phone to go with your laptop and make telephone calls anywhere at any time; just as you can with a mobile, but at a fraction of the cost, and usually free to any other IP phone. This is an excellent inexpensive solution for businesses with a network of offices – all calls between them will be free throughout the world.
Another cool feature is virtual phone numbers. You can get local area numbers allocated to your regular phone number, so that if your family live in LA and you are in New York they can ring your LA local number with their land line and speak to you in New York at local rates.
A VoIP phone is now an attractive option for anyone, both as a cheap alternative to a mobile phone and as a complete replacement for a land-line.

As far as you are concerned, probably not a lot. You can still use your own land-line phone if need be, though in it's simplest form there is no VoIP phone handset as such. Just a microphone and speakers will do. All you need is some software which is supplied by the provider.

As I said, this is a simple example, purely to give a rough idea of how a VoIP phone service sends the data throughout the internet. In practice the VoIP data streams are very much larger as are the packets.
Since the VoIP information has to be in binary form, the signal, of course, has to be a digital one. The digital signal is ‘packetized’ then the packets sent over the internet. The way this is done is to send the signals from one computer to another until it reaches the destination computer, rather like cell phone signals are sent from one antenna to another till they reach the one nearest to you, then from there to your mobile phone receiver.
In fact the packets will take the quickest route through the internet which can sometimes cause problems in that they may arrive at the receiving end in a different order to that in which they were sent. The voice signals are sent as easily as you would send email. In fact VoIP services can completely change the way in which we make phone calls; as revolutionary as the change to mobile cell phones, and many phone companies are wakening up to this and are now selling complete VoIP phone systems for residential users and global corporate companies.
How do you make a phone call using a VoIP service? There are a number of ways:

You can also take your VoIP phone with you, much as you do with your mobile. With most VoIP service providers, however, you can get free calls and the phone is, in effect, your home telephone with your home telephone number. It’s just like carrying your home phone around with you.
Many companies operate a VoIP service on their internal networks, and calls between users on the network are free. Thus the Hong Kong office could call the London office completely free of charge.

Quick response from technical support that illustrates why I'd never suggest that my parents get an internet phone service:
"Some fax machines use the T.38 codec standard, to communication between themselves, the Lingo device does not support this standard. A common symptom of T.38 fax relay problems is a voice call that is established where a fax tone is heard, but the fax negotiation is not completed and the call is eventually dropped. Often this issue is associated with the VoIP - H.323 T.38 Fax Relay Issues. (H.323 is now considered to be the standard for interoperability in audio, video and data transmissions as well as Internet phone and voice-over-IP (VoIP) because it addresses call control and management for both point-to-point and multi-point conferences as well as gateway administration of media traffic, bandwidth and user participation.)"
I'm not taking issue with the response, just pointing out that this is not quite consumer-ready.

VoIP Update
A bit of weirdness with the voice over IP line today - I cannot send a fax over it. The fax machine works on the land line, and phone handsets work on the VoIP line, but no fax sending.
The provider's service has been good & I imagine they'll sort this out for me, but VoIP is, it seems, not quite ready for prime time. I have enough technical aptitude to sort through this stuff, but I can't imagine the generic non tech user putting up with it.

Some time ago I signed up for internet phone service - where your voice gets turned into data that goes out over the internet and then back into the phone network, rather than going via those old fashioned wires. After a few setup hiccups it seems to working nicely and I'm expecting to cut my SBC service back to a bare minimum (for net $$ savings).
Prepaid Phone Cards
I noticed this article in the Washington Post about the plans of Vonage, a VoIP provider (not mine) to offer services via WiFi - basically, you'd have a special handset that works at WiFi hot spots to let you make calls using that internet connection (i.e. not using up your mobile minutes).
It's an interesting idea but I'm kind of skeptical about this particular implementation of it. It means that you have an additional handset to carry, since it doesn't replace your mobile phone. And mobile service is so cheap that frankly it's hard to see that it will be worth it - especially since you will have to pay Vonage for the service. I can see it be useful if you have a WiFi network at your home/office - it could become your "land line" and you could tote it along when you are going to be near a WiFi hotspot so you can make or receive calls on your "land" phone number.

But it seems like a clunkier version of what I've got set up; I use my wireless number as my main number, and for $3 per month I have a service from Cingular that forwards all my wireless calls to my home number when I'm in the house (without using up mobile minutes) - I just pop it into a cradle and it's done.
